Registration Fee Payment

Complete this form to submit the registration fee to reserve your child(ren)'s seat. This is a one-time fee of $35. If you have multiple children, there will be an additional $5 charge per child.

R&L SHUTTLE WILL NOT ISSUE A CREDIT OR REFUND FOR REGISTRATION.
I UNDERSTAND PAYMENTS ARE MADE EVERY TWO WEEKS OR MONTHLY AND MUST BE PAID ON MONDAYS. A $10 LATE FEE WILL APPLY PER DAY. IF NOT PAID BY TUESDAY, TRANSPORTATION WILL NOT BE PROVIDED WEDNESDAY.
I UNDERSTAND IF ANY CHANGES NEED TO BE MADE FOR PICK UP OR DROP OFF ADDRESS, I MUST SUBMIT A NEW FORM.

REGISTRATION FEE CHARGES:

1 child= $35

2 children = $40

3 children = $45

4 children =$50

*add an additional $5 per child.

YOU WILL BE PROMPTED TO ENTER THE AMOUNT AT CHECK OUT. FAILURE TO ENTER THE CORRECT AMOUNT CAN AND WILL RESULT OF FAILURE TO PROCESS REGISTRATION FOR YOR CHILD(REN).
